Incoming comments are processed within a minute and published in accordance with native WordPress comment settings. Postmatic also plays nicely with comment anti-spam plugins (such as Akismet).

If you want comments held for moderation, or to only come from a trusted user, you can control all of that in WP-Admin Settings > Discussion.
